Choosing to have plastic surgery

Choosing to have plastic surgery is an important decision especially breast augmentation surgery. Breast augmentation using saline breast implants or silicone breast implants is one of the most commonly requested aesthetic operations. Breast enhancement surgery is the enlargement of breast volume by the surgical placement of a saline breast implant, silicone breast implant or cohesive gel breast implant ( 410 implant ) .

Most women undergo this type of plastic surgery because they want to feel better about their appearance. Women choose to have breast augmentation surgery to improve their self image. Some feel dissatisfied because their breasts never developed to a size that meets their expectations. Others want to bring balance to a breast that is somewhat smaller than the other. Differences in size between the right and left breasts can be improved with the use of breast implants. Often women want the breast augmentation procedure to restore their natural breast volume, which may have decreased as a result of pregnancy, weight loss, aging or even breast cancer surgery.

During your initial consultation with your local plastic surgeon, you will be encouraged to discuss your desires, fears, expectations, and goals. The first and foremost objective is the creation of breasts that you envision yourself with. In addition to increasing breast size, your plastic surgeon will strive to create a more aesthetically pleasing breast by improving shape, balance, and proportion.

Depending on the desired shape, your plastic surgeon may choose a round or contoured implant for breast augmentation. Round breast implants tend to produce a more rounded appearance to the upper breast. Shaped breast implants are designed to reflect the slope of the breast.

Saline Implants

The human body is composed of about 70% saline. Saline, or salt water is the prime component of all living beings. It is for this reason that Saline implants clearly have the advantage when safety is the first priority. If the implant breaks or leaks, for any reason, it is not harmful to the body. The body will simply absorb the saline, with no harmful side-effects. The contents of the implant are not poisonous to the body, thus making this the safest type of implant to be used.

The saline implants should almost always be placed behind the chest muscle, the Pectoralis Major. This, in addition to the breast itself, will add an additional layer of the patient’s own body tissue over the implant to give a more natural and softer final result. This is where expertise and excellent surgical skill are essential. Los Angeles plastic Surgeon, Dr. Warren Lent, pays special attention to the proper creation of the “pocket” in which the implant will be placed. By creating the correct “pocket”, the results when using saline implants, can be gorgeous. The muscle does not cover the implant on the lower, outer aspect of the breast. Thus, if the breast tissue is very thin the implant can sometimes be palpable in this one spot.

Obviously, if the implant were to leak or break, for any reason, that breast would be smaller. This would require a procedure to replace the damaged implant. This procedure is not as extensive as the original Breast Augmentation, as the old implant can be replaced with a new implant in the same location.

In summary, Saline implants are the safest implants currently in use and with proper surgical technique, a beautiful, stunning, natural appearing and soft breast enhancement can be achieved.

Breast Implants - ‘In Front’ of or ‘Behind’ the Muscle

The main muscle of the front portion of the chest is called the Pectoralis Major. The breast sits directly on top of this muscle. A breast implant can be placed in front of the Pectoralis Major muscle, behind breast tissue alone, or it can be placed behind both the breast and the muscle. The basic goal of all breast augmentation is to have a naturally appearing enlarged breast. The general rule is that the more of the patients own tissue that is available to cover the implant, the better, as this will give a more natural appearance. Therefore, the decision is often based on how much breast tissue the patient is starting with.

The advantage to placing the implant behind the Pectoralis Major muscle is that this adds an extra layer of the patient’s own tissues over the implant. This approach is called the “Submuscular” or “Subpectoral” approach. This technique generally leads to a more natural result, a softer feel and a reduction in the incidence of Capsular Contractures. This technique does require more skill and slightly more operating time in order to create the proper pocket. Of note, is that the muscle does have to be cut for a short distance at its lower most attachments, as this will allow the implant to be positioned correctly. Patients will not even be aware that the muscle was cut, as they will still be able to use their arms normally and perform all activities. The muscle does not cover the implant on the outer or lateral side of the chest, because the muscle does not extend out that far.

Some plastic surgeons still prefer to place the implant above the muscle, behind breast tissue alone. This approach, called the “Submammary” technique, is sometimes used in women who are larger to begin with. This technique is probably best employed when there is a larger volume of breast tissue available to fully cover the implant. This technique is often employed when using a Silicone implant. The disadvantage of the “Submammary” approach is that the implant is usually more easily felt all the way around.

Either technique can give a beautiful result provided that the correct choice is made based on the individual woman’s body and that the surgeon exhibits proper skill in creating the “Pocket” for the implant to sit in.

Pros and Cons of the Inframammary incision

Inframammary incision is made in the natural crease or fold under the breast. The advantage of this incision is that it is placed right in a natural body fold. This aids the healing process as the skin falls together on its own. This incision is particularly good for woman with well-developed “inframammary” creases or folds.

The disadvantage of this incision is for those women who do not have a very well developed inframammary fold. In these women, the scar can be quite visible. Also, when wearing a bathing suit top, if the top rides up at all, such as during sports activity, the scar can show.

Introduction to Breast Augmentation

Quite simply, Breast Augmentation is the process of placing a man-made device behind the breast, to enlarge it. Breast Augmentation, however should be about more than just making the breast larger; it should have several goals:

  1. Making the breast prettier.
  2. Maintaining or improving the shape of the breast.
  3. Improving a woman’s entire figure.
  4. Breast Enlargement

When performed properly, Breast Augmentation can achieve all of these goals. Here in Los Angeles, breast augmentation is extremely popular and is the source of great satisfaction for thousands of women.

Breast Reduction

There is a substantial population of women who have a constellation of symptoms related to overly large breasts. These women experience pain in their upper back, neck and shoulders. This can lead to grooving or indentations in the shoulders from years of pressure by bra straps. Often, overly enlarged breasts, called Macromastia, prevent women from enjoying sports, exercise and other physical activities. Rashes frequently occur under the breasts, especially during warm summer months. Clothes can be ill fitting and there can even be psychological issues from taunting during teenage years. Breast Reduction surgery is a procedure that is designed to relieve women who suffer from these problems.

In Los Angeles breast reduction is a procedure that consistently ranks amongst the highest in patient satisfaction. Candidates for Breast Reduction include those women who have:

  • Excessively large breasts
  • Breasts which sag due to heaviness
  • Shoulder grooving from bra straps
  • Breasts which are disproportionately large for their body frame
  • Chronic rashes in the folds under the breasts
  • Asymmetry of breast size
  • Limitations on physical activity because of breast enlargement

Beverly Hills breast surgeon, Dr. Warren Lent, says that Breast Reduction can be performed at any age once a woman has stopped growing. This usually occurs between the ages of 18 and 20. On rare occasions a teenager may be considered a candidate for surgery prior to completion of her growth phase.
